If I do not qualify for the subsidized loan requested above due to my enrollment level or additional aid received, I authorized Iowa Lakes to process an unsubsidized loan for the same amount.

You must indicate an amount on each line. If you do not wish to borrow or the loan is not listed on your award letter, you must enter 0. Do not enter a word -- it must be a monetary amount.
